Outreach Support Worker
Location: Monmouthshire
Position Type: Full Time (Mon-Fri)

About the Role
As a
Outreach Worker, you will be at the forefront of supporting individuals experiencing homelessness, rough sleeping, or social exclusion. You’ll engage directly with people in the community, often in non-traditional settings, to offer support, build trust, and connect them with vital services.
Your role will focus on:
- Actively engaging with people who are vulnerable, isolated, or hard to reach.
- Providing tailored advice and support around housing, health, benefits, and wellbeing.
- Encouraging individuals to access appropriate services and take steps toward independence.
- Working in partnership with local agencies, charities, health providers, and housing services to ensure wraparound support.
- Maintaining accurate records and contributing to a team approach in delivering high-quality support.
What We’re Looking For
- Experience in outreach, support work, or working with vulnerable adults.
- Strong communication and relationship-building skills, with a non-judgmental approach.
- The resilience and empathy to support people facing complex challenges, including homelessness, mental health issues, substance misuse, and trauma.
- A flexible, proactive attitude with the ability to work independently and as part of a team.
- Knowledge of housing, safeguarding, and local support networks is desirable.
- A full UK driving licence (and access to a car) is essential.
