Job Summary
We are seeking a dynamic and compassionate Domiciliary Care Coordinator to lead and coordinate home care services for our service users and carers.
The ideal candidate will have a background in domiciliary care and a strong customer care focus. Be organised and have the ability to work under pressure and prioritise their work load, with excellent communication skills, clear and concise language (written and spoken). They must have knowledge and experience working in Domiciliary care sector and have knowledge/experience using rostering systems.
They will also need to be responsible and able to deal with situations efficiently and in a timely manner.
Duties will include;
- To coordinate service user and care worker visits/calls
- Prioritise tasks to meet changes and unplanned emergencies
- Complete rotas on a weekly basis
- Arrange cover for care workers annual leave, sickness and general absenteeism
- To work in partnership with the risk/field assessors
- Ensure service users are matched to the right care worker
- Participate in on call duties as pro rata
- Liaise with and work in partnership with health and social care professionals as well as other allied professionals and stakeholders
- Use of rostering systems and electronic monitoring
- Responsible for the day to day management of care call planning
- Amending rostering system and making necessary changes in a timely manner
- Carry out assessments and care calls during shortfalls and emergencies
- Supervise and support care staff, providing guidance, training, and performance feedback
- Maintain clear communication with clients, families, healthcare professionals, and team members to ensure seamless care delivery
- Ensure compliance with health and safety regulations, safeguarding policies, and care standards
- Excellent communication skills to liaise effectively with clients, families, and team members
- Familiarity with IT systems used in healthcare or social care settings for documentation and reporting purposes
- Ability to handle sensitive situations with empathy, professionalism, clarity and confidentially.
Pay: £27,500.00-£28,800.00 per year
