Care Coordinator – Community Lead
Location: Wellingborough, Kettering & Rushden | Office & Community Based
Hours: Full-time, Monday to Friday with mandatory alternate weekends
Contract: Permanent
On-Call: Participation in an on-call rota with senior management support
Northants Community Care is a local provider delivering high-quality domiciliary, supported living, live-in, and 24/7 care services across Wellingborough, Kettering, and Rushden. We are seeking an experienced Care Coordinator – Community Lead to support the safe and effective delivery of care services, provide leadership to care staff, and support the operational running of the service.
The Role
You will play a key role in coordinating care delivery across the community, leading by example in line with our values, and supporting both care staff and management to maintain high standards of person-centred care.
Key Responsibilities
- Coordinating and maintaining staff rotas across domiciliary, supported living, live-in, and 24/7 care services
- Responding to staff absence and service changes to ensure continuity of care
- Participating in the on-call rota alongside senior management, responding to out-of-hours issues and emergencies
- Supporting and supervising care staff in the community through spot checks, observations, and mentoring
- Acting as a role model, promoting professionalism, reliability, and compassionate practice
- Supporting quality assurance, incident reporting, safeguarding, and compliance processes
- Maintaining accurate records using digital care planning and rota systems
- Supporting the Business Manager by preparing hours for payroll, invoicing, and mileage submissions
- Ensuring accuracy, confidentiality, and timeliness of operational data
- Communicating effectively with clients, families, and external professionals
- Undertaking care calls when required to support safe service delivery
About You
- Experience in domiciliary or supported living care at senior or supervisory level
- Strong understanding of CQC standards and regulatory compliance
- Values-led, organised, and confident in leading others
- NVQ/QCF Level 3 in Health & Social Care (Level 5 desirable)
- Full UK driving licence and access to a vehicle
