Job Overview
We are seeking a compassionate and dedicated Rehabilitation Support Worker to join our team. The successful candidate will play a vital role in supporting individuals with brain injuries. Applicants must be committed to providing a genuine desire to make a positive difference in people's lives.
Duties
. To assist people with brain injuries along a journey of recovery and rehabilitation to maximise independence and quality of life
· To understand the needs of the clients and agree a support plan
· To understand the risk management process including identifying safeguarding concerns and supporting risk assessments
· Plan and organise activities for key work clients 1:1 and groups in line with their support plans
. Provide personal care to support well-being and independence of clients
· Monitor progress, reporting and updating the support plans on a regular basis
· To record attendance, activities and feedback using Headway Swindon’s IT system
· To work as an effective member of a supportive and cohesive team.
· To adhere to Headway Swindon’s policies and procedures.
Skills
- Proven experience working in Social Care settings.
- Experience with brain injury survivors is highly desirable.
- Excellent organisational skills for managing key work clients' needs effectively.
- Good literacy, IT and numeracy skills.
- Excellent communication skills
- Effective behaviour management skills to handle challenging situations calmly and professionally.

Education:
- GCSE or equivalent (required)
Experience:
- Social Care Work: 2 years (preferred)
