Overview
We are seeking a dedicated Care Navigator / Administrator to join our healthcare team. This vital role involves coordinating patient care, managing administrative tasks, and ensuring smooth communication between patients and healthcare providers. Responsibilities
- Act as the primary point of contact for patients, providing guidance and support throughout their care journey
- Organise appointments, follow-ups, and referrals in accordance with healthcare protocols
- Maintain accurate and up-to-date patient records using electronic health record (EHR) systems
- Coordinate with multidisciplinary teams to facilitate seamless patient care delivery
- Manage administrative duties such as data entry, filing, and correspondence
- Assist in preparing reports and documentation for internal and external stakeholders
- Ensure compliance with data protection regulations and confidentiality standards
- Support the continuous improvement of administrative processes to enhance efficiency
Experience
- Prior office or administrative experience is essential, preferably within a healthcare or social care setting
- Familiarity with healthcare systems, patient management software, or EHR platforms is advantageous
- Excellent organisational skills with the ability to prioritise tasks effectively
- Strong verbal and written communication skills in British English
- Ability to work independently as well as part of a team in a fast-paced environment
