About the Role As a Community Supported Living Support Worker, you will provide person-centred support to adults with learning disabilities, autism, mental health needs, physical disabilities, acquired brain injuries, complex support needs, behaviours of concern and sensory impairments.
Our aim is to support people to live as independently as possible within their own homes while achieving their personal goals, maintaining relationships and taking an active role in their local communities.
What You Will Be Doing Every day is different, but your role may include:
- Supporting people to live independently within their own homes.
- Assisting individuals to achieve personal goals and outcomes.
- Encouraging participation in community activities, hobbies, education, employment and volunteering opportunities.
- Supporting with daily living skills including cooking, shopping, budgeting and household tasks.
- Providing personal care when required.
- Supporting individuals to attend appointments and access local services.
- Promoting choice, dignity, rights and independence at all times.
- Providing emotional reassurance, encouragement and companionship.
- Supporting people to develop confidence, communication and life skills.
- Maintaining accurate records using digital care management systems.
