Responsibilities
- Support patients or residents with personal care tasks such as bathing, dressing, grooming, and mobility assistance
- Assist with medication management according to care plans and instructions from healthcare professionals
- Maintain accurate records of care provided using IT systems and ensure compliance with confidentiality standards
- Communicate effectively with patients, residents, family members, and healthcare team members to ensure clear understanding of needs and progress
- Support the development and implementation of personalized care plans tailored to individual requirements
- Drive or assist with transportation needs for appointments or outings when required
- Contribute to a safe environment by adhering to health and safety protocols and reporting any concerns promptly
Skills
- Excellent communication skills in English, with the ability to listen actively and convey information clearly
- Strong IT literacy for documenting care activities, updating care plans, and using relevant healthcare software
- Valid driving license is preferred for supporting home care visits or transportation duties
- Experience working in a care home setting or providing home care services is advantageous
- Knowledge of care plans, medication procedures, and health & safety standards in healthcare environments
- Compassionate attitude with a genuine desire to support others’ well-being
- Good organizational skills to manage multiple responsibilities efficiently
