SUPPORT WORKER – DAY CENTRE & COMMUNITY ACTIVITIES

My Health Care Support Ltd is recruiting Support Workers to help people take part in meaningful day-centre and community activities in Stoke-on-Trent.
ABOUT THE ROLE
You will provide respectful, person-centred support that builds on each person’s strengths, choices and goals. The work may include support at a day-centre setting and at agreed community locations. Hours are offered according to service needs; this advert does not guarantee a minimum number of weekly hours.
KEY RESPONSIBILITIES
- Support people to choose and take part in meaningful activities, hobbies and community opportunities
- Encourage independence, confidence, social inclusion and positive relationships
- Provide daily-living support in line with individual support plans, assessed needs and preferences
- Help people travel safely to agreed activities and appointments where this forms part of their support
- Observe wellbeing and report changes, concerns or incidents promptly
- Maintain clear, accurate and timely records
- Follow safeguarding, confidentiality, health and safety, risk-management and company procedures
- Work reliably with colleagues, families and relevant professionals
- Support medication only when trained, assessed as competent and authorised under the person’s support plan and company procedure
ESSENTIAL CRITERIA
- A respectful, patient and strengths-based approach
- Clear communication and the ability to build professional, trusting relationships
- Reliability, good judgement and a commitment to safeguarding
- Ability to follow support plans, risk assessments and recording requirements
- Ability to travel reliably to work and agreed community locations in Stoke-on-Trent
- Existing right to work in the UK
DESIRABLE CRITERIA
- Experience in supported living, day services, domiciliary or community care, residential care, healthcare support, learning-disability services or mental-health support
- Experience planning or supporting community participation and meaningful activities
- Care Certificate, Level 2 or Level 3 Health and Social Care qualification, or equivalent relevant training
- Full UK driving licence and access to reliable transport
PAY AND HOURS
Pay is £12.71–£14.00 per hour. The rate offered will reflect relevant experience, qualifications, duties and shift requirements. Opportunities are part-time and zero-hours, with variable hours based on service needs and availability.
