· Job Title: Domiciliary Care Coordinator
· Reports To: Registered Manager
· Hours: Full-Time (including participation in on-call rota)
· Salary: Negotiable, dependent on skills and experience
2. Job Purpose
The Domiciliary Care Coordinator is responsible for planning, organising, and monitoring the delivery of high-quality domiciliary care services. The Domiciliary Care Coordinator acts as the deputy to the Registered Care Manager, and will step up when the need arises. The role ensures clients receive safe, person-centred support, staff are deployed effectively, and all care delivery complies with CQC, Safeguarding, Local Authority and organisational standards.
3. Key Responsibilities3.1 Care Planning & Client Management
· Complete initial assessments, risk assessments, and care reviews.
· Develop and maintain person-centred care plans.
· Respond promptly to changes in client needs.
· Maintain positive relationships with clients, families, and professionals.
3.2 Staff Coordination & Scheduling
· Prepare efficient rotas ensuring full visit coverage.
· Allocate care workers based on skills and client needs.
· Manage last-minute changes due to sickness, emergencies or changing Service User needs.
· Provide guidance and supervision to care staff.
3.3 Compliance & Quality Assurance
· Ensure care delivery meets CQC, Safeguarding, Local Authority and organisational standards.
· Maintain accurate documentation and records.
· Conduct spot checks, audits, and client reviews.
· Promote safeguarding and report concerns appropriately.
3.4 Communication & Liaison
· Act as main contact for clients, families, staff, and external agencies.
· Work collaboratively with the Registered Manager.
· Attend meetings, training, and supervision.
· Build positive relationships with Service Users, their families and other professionals and organisations, promoting ASRA, with a view to expand service delivery.
3.5 Operational Support
· Assist with recruitment and onboarding and continuous staff development.
· Participate in the on-call rota.
· Carry out field visits when required.
4. Qualifications & Experience4.1 Essential
· NVQ Level 2 or 3 in Health & Social Care (or working towards).
· Experience in domiciliary or community care.
· Full UK driving licence and access to a vehicle.
· Understanding of safeguarding and person-centred care.
· Experience working in domiciliary or community care.
· Experience completing or contributing to care plans, assessments, or reviews.
· Experience working in a fast-paced environment with changing priorities.
4.2 Desirable
· NVQ Level 3 in Leadership & Management.
· Experience supervising care staff.
· Knowledge of local authority commissioning.
· Experience conducting care reviews and risk assessments.
· Experience supporting or coordinating care staff.
