Overview
We are seeking a dedicated and compassionate Practice Nurse to join our healthcare team. The ideal candidate will play a crucial role in the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.
Responsibilities
a. Develop, implement and embed health promotion and well-being programmes
b. Support the clinical team in the development of individual treatment plans for chronic disease patients
c. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patient’s health
d. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
e. Provide wound care (ulcer /Doppler etc.) to patients
f. Provide travel medicine services
g. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
h. Maintain accurate clinical records in conjunction with extant legislation
l. Ensure read codes are used effectively
j. Maintain chronic disease registers
k. Develop, implement and embed well woman clinics
l. Chaperone patients where necessary
m Support the team in dealing with clinical emergencies
n Implement vaccination programmes for adults and children
Requirements
- Registered Nurse with current NMC registration
- Strong knowledge of anatomy and physiology.
- Previous experience in a clinical setting is essential.
- Excellent communication and interpersonal skills to interact effectively with patients and colleagues.
- Ability to work independently as well as part of a team.
- Strong organisational skills with attention to detail in documentation and patient care processes.
- Knowledge of clinical governance
