Care Management Assistant
The Care Management Assistant role is to help promote independence and safeguard older and disabled adults in Carmarthenshire who appear to have care and support needs. This position is in the Carmarthen Intake Team, focusing on adults with physical disabilities and older people with cognitive challenges. Based in Carmarthen, you'll be assisting residents from Carmarthen Town, Towy, Teifi, and Taf areas.

The postholder will undertake assessments, provide information, advice and assistance, contribute to care and support planning, and complete reviews in line with the principles of the Social Services and Well-being (Wales) Act and other relevant legislation.
Applicants should be motivated and committed individuals with experience of working within social care or related services, and with older or disabled people. They will need to demonstrate effective communication and interpersonal skills, the ability to work as part of a team, and the ability to engage with individuals and their carers.
Applicants should have knowledge of the issues facing Social Services, particularly in relation to older people and people with physical disabilities. They should also have an understanding of legislation and statutory obligation, policy and practice relating to assessment, reablement and care management, and be able to support individuals to identify what matters to them and achieve their personal outcomes.
The ability to travel within the county is essential.
Welsh language skills are required for this post: Welsh spoken Level 2 and Welsh written Level 1. Support can be provided on appointment to reach the required level where appropriate.
An Enhanced Disclosure and Barring Service check for the Adult Workforce will be required for this post, as the role involves visiting vulnerable people.
