Community Team Leader
North West: Manchester Area (Field-Based Role)
Salary: £30,000 per annum + £5,000 car allowance
Hours: 36 hours per week with 12 hours on shift and 24 hours Management and Admin
We're looking for a motivated, compassionate, and proactive Community Team Leader to join our growing Complex Care Team, supporting a range of services across the Greater Manchester area.
In this rewarding role, you will work closely with the Complex Care Manager (CCM) to ensure our Support Workers are fully equipped, supported, and empowered to deliver outstanding, person-centred care. You will play a key role in maintaining high standards of service delivery, promoting staff engagement and retention, and ensuring all care packages remain safe, effective, and compliant with regulatory requirements.
As a natural relationship builder, you will be confident working with the people we support, their families, healthcare professionals, and colleagues across the business. You will champion a culture of quality, accountability, and continuous improvement, while ensuring our values and commitment to person-centred care remain at the heart of everything we do.
This is a varied and fast-paced position that requires excellent communication and organisational skills, a flexible approach, and a genuine passion for making a positive difference in people's lives.
Key Responsibilities
Recruitment & Onboarding

Support the recruitment process by attending interviews and assessing candidates.

Coordinate and attend introductions between new Support Workers and the individuals they will support.

Build strong relationships with Recruitment Partners to ensure accurate and timely communication.

Support new starters through their onboarding journey, conducting weekly check-ins to ensure a positive induction experience.
Quality & Service Delivery

Support the Complex Care Manager in maintaining high standards of care and service delivery.

Assist with rota preparation and service planning when required, ensuring all care packages are adequately staffed.

Attend multidisciplinary team meetings, safeguarding meetings, and operational reviews as required.

Conduct regular reviews of contingency plans to ensure care packages remain safe, effective and person-centred.

Monitor and promote compliance with CQC standards, internal policies, and regulatory requirements.
Staff Support & Development

Provide regular supervision and performance support to Support Workers.

Identify training and development needs and ensure staff have the skills and knowledge required to perform their roles effectively.

Support employee engagement initiatives and contribute to staff retention strategies.

Conduct development reviews and assist with succession planning across the team.
Experience & Skills Required of a Community Team Leader
Essential
-
Previous experience in a leadership, supervisory or team leader role within Health & Social Care.

Experience supporting individuals with complex care needs in community-based settings.

Knowledge of CQC regulations, safeguarding processes, and person-centred care principles.

Experience conducting staff supervisions, coaching or performance reviews.

Strong organisational skills with the ability to manage multiple priorities.

Excellent communication and relationship-building skills.

Ability to work independently and make informed decisions.

Full UK driving licence and willingness to travel across Stockton-on-Tees, Durham and North Yorkshire.
Desirable

Level 3 Diploma in Health & Social Care (or equivalent).

Experience within Complex Care, Supported Living, Domiciliary Care or Community Care services.

Experience supporting recruitment, onboarding and workforce planning activities.

Leadership or management qualification.
