Job Summary
We are seeking a compassionate and proactive Care Manager to lead and coordinate comprehensive care services for our senior residents. In this vital role, you will develop personalized care plans, oversee daily patient needs, and ensure the highest quality of life for individuals with diverse health conditions, including dementia. The Care Manager will serve as a key liaison between residents, families, healthcare providers, and staff, ensuring seamless communication and exceptional care delivery.
Responsibilities
- Assess residents’ health status, needs, and preferences to create tailored care plans that promote independence and well-being.
- Coordinate medical appointments, medication administration, and treatment schedules in collaboration with healthcare professionals.
- Monitor resident progress regularly, adjusting care strategies to meet evolving needs and behaviors.
- Lead efforts in behavior management and dementia care to enhance residents’ comfort and safety.
- Oversee daily caregiving activities, including meal preparation, mobility assistance, and light housekeeping tasks.
- Ensure compliance with all safety protocols, including first aid procedures and heavy lifting techniques.
- Maintain detailed documentation of care plans, incident reports, and medication logs to support case management efforts.
Skills
- Proven experience in nursing home or assisted living environments with a focus on senior care and dementia management.
- Strong knowledge of medical terminology, hospice care principles, and medication administration protocols.
- Excellent caregiving skills with an emphasis on patient-centered care and behavior management strategies.
- Ability to handle heavy lifting safely while maintaining a positive attitude during physically demanding tasks.
- Valid driver’s license with reliable transportation for home care visits when necessary.
- Familiarity with case management practices to coordinate comprehensive services effectively.
- Exceptional communication skills to collaborate with families, healthcare teams, and residents’ support networks.
