Job Description – Transformation Manager

About Homeless Action in Barnet
Homeless Action in Barnet (HAB) has supported people experiencing homelessness for over 25 years. We provide a wide range of services including a day centre, emergency and supported accommodation, winter shelter, housing support, food, showers, advice and long-term recovery support.
We are now entering an exciting new phase of development, working with a wide range of external support providers and investing in more joined-up, person-centred services that help people not simply survive homelessness, but move towards lasting independence.
Purpose of the Role
The Transformation Manager will lead the development and implementation of a new, integrated model of support for people experiencing homelessness in Barnet.
Working across HAB and alongside statutory and voluntary sector partners, the postholder will improve how organisations work together to ensure clients receive coordinated, trauma-informed support that addresses housing, health, wellbeing, employment and recovery together.
The role will establish new systems, partnerships and ways of working that improve outcomes for clients whilst making partnership working and support services more efficient.
Key Responsibilities
Service Transformation
- Lead the implementation of HAB's transformation programme.
- Design and embed new client pathways that provide coordinated, holistic support.
- Introduce continuous improvement processes across services.
- Identify opportunities to improve efficiency, quality and client outcomes.
Multi-Agency Partnership Working
- Build strong relationships with local authority teams, NHS services, mental health providers, substance misuse services, probation, employment providers and voluntary organisations.
- Chair regular Multi-Disciplinary Team (MDT) meetings, bringing partners together around clients with complex needs.
- Develop shared referral pathways and clear escalation processes.
- Improve communication and joint working between agencies.
- Represent HAB at strategic partnership meetings.
Client Coordination
- Oversee the development of coordinated care plans for clients with multiple support needs.
- Ensure every client has a named lead worker and clear support plan.
- Monitor progress through regular multi-agency reviews.
- Remove barriers preventing clients accessing appropriate services.
- Promote psychologically informed and trauma-informed approaches across all services.
Systems & Quality Improvement
- Develop effective case management and monitoring systems.
- Improve data collection and information sharing arrangements.
- Produce reports demonstrating outcomes and service impact.
- Use data to identify trends, gaps and opportunities for service improvement.
- Ensure appropriate governance and confidentiality procedures are maintained.
Leadership
- Support staff to embed new ways of working.
- Line Manage Centre Coordinator and Projects Coordinator
- Coordinate training and guidance on partnership working and trauma-informed practice.
- Work closely with the CEO/SMT to implement organisational strategy.
- Cover for CEO in their absence
Person Specification
Essential
- Excellent partnership-building skills with statutory and voluntary sector organisations.
- Experience coordinating complex, multi-agency support.
- Strong understanding of homelessness and the challenges faced by people with multiple and complex needs.
- Knowledge of trauma-informed and psychologically informed practice.
- Excellent organisational and project management skills.
- Ability to analyse data and use evidence to improve services.
- Strong communication and influencing skills.
- Experience developing policies, procedures or new service models.
- Commitment to equality, inclusion and person-centred practice.
Desirable
- Experience managing or leading service transformation within homelessness, housing, health, social care or a related sector.
- Experience chairing multi-agency meetings.
- Knowledge of Barnet's homelessness landscape.
- Experience of improving services through partnership working.
- Understanding of housing legislation and homelessness policy.
- Project management qualification or equivalent experience.
Success Measures
Within the first 12 months the postholder will have:
- Established weekly MDT meetings involving key partner agencies.
- Introduced shared care planning for clients with the highest levels of need.
- Improved referral pathways between HAB and partner organisations.
- Developed robust monitoring systems to evidence outcomes.
- Increased engagement with health, housing and recovery services.
- Improved client progression into stable accommodation and longer-term independence.
- Helped to foster a culture of collaborative, trauma-informed working across HAB.
