To provide specialist medication reviews, prescribing advice, and evidence-based treatment recommendations for adults within Hull Community Mental Health Teams (CMHTs) and Primary Care Mental Health Networks (PCMHNs). Using a person-centred approach, the post holder will assess, monitor, and manage complex mental health needs, identifying appropriate treatment options.
To deliver Non-Medical Prescribing (NMP) clinics across CMHT and PCMHN services, undertaking assessment, medication initiation, review, and optimisation. The post holder will work with the multidisciplinary team to develop holistic care plans that support recovery and improve outcomes.
To support Consultant Psychiatrists, Clinical Leads, and operational teams in meeting key performance indicators and ensuring compliance with NICE guidance, Trust policies, and local standards. This includes supporting the safe monitoring of Clozapine, long-acting injectable antipsychotics, and Sodium Valproate, ensuring physical health monitoring, risk management, and annual reviews are completed in line with Trust requirements.

Provide specialist assessment, prescribing, medication reviews and medicines optimisation for adults receiving care within Hull Community Mental Health Teams (CMHTs) and Primary Care Mental Health Networks (PCMHNs), ensuring care is delivered in line with evidence-based practice, NICE guidance and Trust policies.
Deliver dedicated Non-Medical Prescribing (NMP) clinics, undertaking comprehensive clinical assessments, initiating and reviewing treatment plans, and monitoring the effectiveness and safety of prescribed medication.
Work collaboratively within the multidisciplinary team to develop and implement holistic, person-centred care plans incorporating pharmacological and non-pharmacological interventions to support recovery and positive clinical outcomes.
Provide expert advice and consultation to clinicians, care coordinators, GPs and partner agencies regarding psychotropic medication, medicines optimisation and safe prescribing practice.
Support service performance, quality standards and key performance indicators. Lead on the monitoring and review of high-risk medicines, including Clozapine, long-acting injectable antipsychotics and Sodium Valproate, ensuring compliance with local and national safety requirements.
Contribute to service development, audit and quality improvement initiatives, while providing clinical leadership, supervision and support to colleagues and students.

Working autonomously within their scope of professional practice, the post holder will deliver high-quality, evidence-based interventions that support recovery, improve outcomes and promote safe and effective use of medication.
The post holder will:
Undertake comprehensive mental health assessments for individuals presenting with a range of severe, enduring and complex mental health needs.
Independently prescribe, review, monitor and optimise medication in accordance with professional standards, NICE guidance, Trust policies and local prescribing formularies.
Deliver dedicated Non-Medical Prescribing (NMP) clinics across Hull CMHT and PCMHN services, providing timely assessment, treatment, review and follow-up interventions.
Develop and implement person-centred treatment plans in partnership with service users, carers and the wider multidisciplinary team, ensuring shared decision-making is embedded within practice.
Monitor the effectiveness of prescribed treatments, assessing therapeutic outcomes, side effects, adherence and physical health considerations, making appropriate changes to treatment where required.
Provide specialist advice and consultation to clinicians, care coordinators, primary care colleagues and partner agencies regarding psychotropic medication, medicines optimisation and complex prescribing decisions.
Support the safe management and monitoring of high-risk medications, including Clozapine, long-acting injectable antipsychotics and Sodium Valproate, ensuring compliance with national and local safety standards.
Contribute to the physical health monitoring agenda for individuals receiving mental health services, ensuring appropriate investigations, monitoring and follow-up are completed.
Liaise effectively with General Practitioners, community pharmacies, secondary care services and other stakeholders to ensure continuity of care and safe prescribing arrangements.
Participate in multidisciplinary team meetings, case discussions and clinical reviews, contributing specialist prescribing knowledge to support clinical decision-making.
Maintain accurate, contemporaneous and legally compliant clinical records using Trust electronic patient record systems.
Identify and manage clinical risk, safeguarding concerns and medication-related incidents in accordance with Trust policy and professional standards.
Contribute to service development, audit, quality improvement initiatives and the implementation of evidence-based practice across Hull Mental Health Planned Care Services.
Provide clinical leadership and support to colleagues, promoting best practice in prescribing, medicines management and patient safety.
Offer supervision, mentorship and support to registered practitioners, students and colleagues undertaking Non-Medical Prescribing training where appropriate.
Maintain professional registration, continuing professional development and competency in Non-Medical Prescribing practice, ensuring compliance with NMC standards and Trust requirements.
Promote the Trust values of Caring, Learning and Growing whilst contributing to the delivery of safe, effective and high-quality mental health services across Hull.
