Job Overview
We are seeking a dedicated and knowledgeable Trusted Assessor to join our team at Eleanor Care. The successful candidate will play a vital role in evaluating service users' needs, ensuring they receive appropriate care solutions tailored to their individual requirements. This position offers an opportunity to make a meaningful difference by supporting vulnerable individuals through comprehensive assessments grounded in clinical expertise and compassionate understanding.
Responsibilities
- Conduct thorough assessments of service users to determine their care needs, focusing on patient care and functional abilities..
- Collaborate closely with multidisciplinary teams, families, and external agencies to develop personalized care plans.
- Provide clear guidance on suitable care packages, equipment, or adaptations required to enhance quality of life.
- Maintain detailed documentation of assessments in compliance with regulatory standards and organizational policies.
- Support the onboarding process by ensuring new service users receive timely and appropriate support arrangements.
- Stay informed about best practices in social and health care assessment methodologies to continually improve service delivery.
Qualifications
- Proven experience in care with a strong understanding of care needs and health conditions relevant to social care assessments.
- Excellent communication skills, capable of engaging effectively with service users, families, and multidisciplinary teams.
- Demonstrated ability to conduct detailed assessments with attention to individual needs and preferences.
- Knowledge of current social care legislation, safeguarding procedures, and regulatory requirements.
- Ability to work independently while maintaining collaborative relationships within the team.
- A relevant qualification in health or social care is preferred; additional training in assessment techniques is advantageous. However training will be given.
