Job Description Summary (Main duties and responsibilities)
The successful candidate will work as part of a team supporting those at risk of developing a psychosis, working as part of a Psychologically minded and led service and holding a caseload of service users (at present around 15 for a full time member of staff)
To help develop this new and exciting service and to expand the role of a band 6 within the team.
You must be an NMC Registered Mental Health or Learning Disability Nurse, Occupational Therapist of Social worker with the appropriate qualifications and registration to apply for this post.
You must also hold a full valid driving licence for use in the UK and have access to suitable vehicle, and be willing to convey service users from time to time.

To Delivery of Effective Healthcare within the organisation
Be responsible for the assessment, planning, implementation and evaluation of programmes of evidenced based care to a group of service users according to changing healthcare needs and varying levels of complexity.
Use advanced skills and expert knowledge to assess the mental health and social needs of a defined client group, instigating therapeutic care plans and treatments based on the best available evidence in order to improve health outcomes
Work within a multi-disciplinary team to ensure effective care plans and treatments are in place for complex services users. This will involve co-ordinating care and treatment activities with other professionals or external agencies, including detailed planning of treatment and discharge plans to ensure the best outcomes for patients/service users
