Raise concerns with regards to babys development/physical health with the Lead Practitioner and families Primary care team including Community health visiting services. To attend multi-disciplinary team meetings, multi-professional reviews and child protection case conferences, to report on the identified difficulties between parent/infant and the interventions being offered.
