Job Purpose
We are looking for an experienced, organised and proactive Supported Living Care Coordinator to help oversee the day-to-day running of four supported living houses across East Lancashire.
Working closely with the Registered Manager and Supported Living Manager, the Care Coordinator will play a key role in ensuring that people receiving support are provided with safe, effective, person-centred and high-quality care and support.
This is a hands-on position. The successful candidate must be comfortable moving between services, supporting staff teams and providing direct support to individuals when required.
The Care Coordinator will help ensure that support plans are developed, implemented and regularly reviewed and that staff understand how to provide support in accordance with each person’s individual needs, wishes, choices and outcomes.
Key ResponsibilitiesService Coordination and Oversight
- Assist the Registered Manager and Supported Living Manager with the effective day-to-day running of four supported living houses.
- Maintain a regular presence across all four services and develop positive working relationships with the people we support and staff teams.
- Help ensure each service is safe, well organised and operating in accordance with company policies and procedures.
- Identify operational issues and work with management and staff to resolve them promptly.
- Support effective communication between the supported living houses and the management team.
- Assist with ensuring adequate staffing levels are maintained across the service.
- Respond appropriately to unexpected staffing issues, incidents and changes in people’s support needs.
- Promote consistency and high standards across all four houses.
Support Plans and Person-Centred Support
- Develop, implement, monitor and review high-quality person-centred support plans.
- Ensure support plans accurately reflect each person’s current needs, preferences, communication needs, abilities, risks, goals and desired outcomes.
- Work directly with individuals to ensure they are involved as much as possible in decisions about their own lives and support.
- Ensure staff understand support plans and are able to implement them consistently.
- Identify changes in an individual’s needs and ensure these are communicated, documented and acted upon.
- Contribute to reviews involving individuals, families, advocates, social workers and other professionals where appropriate.
- Help individuals develop and maintain independence, choice, control and community involvement.
Hands-On Support
This is not solely an office-based role.
The Care Coordinator will be expected to:
- Provide direct, hands-on support to individuals across any of the four supported living houses when required.
- Work alongside staff to model good practice.
- Provide cover and additional support during periods of increased need, staff absence or unexpected situations where reasonably required.
- Support individuals with daily living skills, appointments, activities and community access.
- Provide personal care where this forms part of an individual’s assessed support needs.
- Respond calmly and professionally to challenging or emergency situations.
- Be willing to work flexibly across the service according to operational needs.
Staff Support and Leadership
- Provide guidance, support and day-to-day direction to Support Workers and Senior Support Workers.
- Encourage positive teamwork and professional communication.
- Help new staff understand the needs of the people they are supporting.
- Assist with staff inductions and competency development.
- Identify areas where staff require additional training, guidance or support.
- Challenge poor practice appropriately and report concerns to management.
- Promote a positive culture in which individuals receiving support are treated with dignity, respect and as equal members of their communities.
- Assist management with supervision, observations and staff development where required.
Quality and Compliance
- Assist management in maintaining high standards throughout the supported living service.
- Carry out regular checks and audits as delegated by management.
- Ensure records are accurate, professional, current and completed to the required standard.
- Monitor support documentation and ensure required reviews are completed.
- Support compliance with safeguarding procedures, Mental Capacity Act principles, health and safety requirements and relevant care regulations.
- Report safeguarding concerns, incidents, accidents and significant changes promptly and appropriately.
- Assist with investigations and follow-up actions where required.
- Promote learning from incidents and encourage continuous improvement across the service.
- Maintain confidentiality and appropriate professional boundaries at all times.
Health, Medication and Risk Management
- Help ensure individual risk assessments are appropriate, current and followed by staff.
- Promote positive risk-taking while ensuring individuals and staff are appropriately safeguarded.
- Support good medication practice in accordance with company policies and individual requirements.
- Escalate medication errors or concerns promptly.
- Help ensure health appointments and professional recommendations are followed up appropriately.
- Work with external health and social care professionals when required.
Essential Requirements
Applicants must:
- Hold a full driving licence and be able to travel between supported living services across East Lancashire.
- Have previous experience working within supported living, social care or a similar care environment.
- Have experience supporting people with learning disabilities, autism and/or other additional support needs.
- Have the ability to develop, implement and review person-centred support plans.
- Be willing and able to provide hands-on care and support when required.
- Have a good understanding of safeguarding and the responsibilities of staff working in adult social care.
- Have good written communication skills and be able to maintain accurate professional records.
- Have strong organisational and time-management skills.
- Be confident communicating with individuals, families, staff, managers and external professionals.
- Be able to work independently while also being an effective member of the management and wider support team.
- Be flexible and willing to work across all four supported living houses.
- Be able to respond appropriately to changing priorities and unexpected situations.
Desirable Requirements
It would be advantageous for applicants to have:
- Previous experience as a Care Coordinator, Senior Support Worker, Team Leader or Deputy Manager.
- Experience overseeing more than one supported living service.
- A Level 3 qualification in Health and Social Care or equivalent.
- Knowledge of CQC expectations and the principles of safe, effective, caring, responsive and well-led services.
- Experience of staff supervision and competency assessment.
- Experience completing audits, risk assessments and service quality checks.
- Experience working with multidisciplinary teams, social workers, families and other professionals.

Accountability
The Supported Living Care Coordinator is responsible to the Registered Manager and Supported Living Manager.
The post holder will work within the boundaries of their role and delegated authority. Significant safeguarding, staffing, health, behavioural, medication or operational concerns must be escalated promptly to the appropriate manager.
Overall Expectation
The purpose of this position is to provide a strong operational link between the management team and the four supported living houses.
The Care Coordinator should be visible within the services, know the individuals being supported, understand their support plans, know the staff teams and be able to identify where improvements or additional support are required.

Pay: £14.25 per hour
