Summary
Join IHI Care Services as a Care Manager in our home care team based in Kings Heath, Birmingham. In this vital role, you will coordinate and oversee personalised care plans, ensuring our clients receive compassionate support that aligns with our mission of enhancing quality of life. Hours of work are 9am to 4pm, Monday to Friday (35 Hours per Week). Some on-call cover will also be required.
Responsibilities
- Develop and implement individualised care plans tailored to each client’s needs.
- Monitor patient progress and adjust care strategies accordingly.
- Manage a team of care coordinators.
- Ensure deadlines are met.
- Collaborate with families, healthcare professionals, and support teams to ensure comprehensive care.
- Maintain accurate documentation of all care activities.
- Manage staff rotas.
- Deal with Social Services in regards to reviews, new package starts, safeguarding's etc.
- Maintain compliance with CQC regulations.
- Through successful management provide the highest level of care and compassion to all our clients.
- Available as Part time, leading to Full time.
Qualifications
- Proven experience in managing a home health, assisted living, or related caregiving setting.
- Excellent communication skills and ability to work empathetically with diverse populations.
- Ability to work to timelines.
- Ability to manage an office based team
- Ability to manage care staff in the community.
- NVQ Level 5 in Health and Social Care.
