JOB SUMMARY
We are seeking an experienced Mental Health Manager to join our Liaison Care Transformation team, where you lead complex clinical assessments, case reviews and package redesign activity across a range of health and care workstreams. Using specialist knowledge of Continuing Healthcare (CHC) and Mental Health services, you will undertake assessments, provide evidence-based recommendations, support commissioning decisions and help ensure individuals receive safe, effective and person-centred care.
This role requires a confident and autonomous clinician with strong professional judgement, excellent case management skills and the ability to deliver high-quality outcomes within a fast-paced and evolving environment.
PRINCIPAL TASK AND ACTIVITIES

Undertake complex Continuing Healthcare (CHC) assessments, reviews and case evaluations, applying the National Framework and relevant guidance to support robust clinical decision-making.

Lead the review, analysis and redesign of complex packages of care, ensuring recommendations are clinically appropriate, outcome-focused and represent value for money.

Complete comprehensive clinical assessments and produce high-quality reports with clear rationale, evidence-based recommendations and defensible professional judgement.

Apply advanced clinical reasoning to evaluate individuals' health needs, levels of risk and support requirements, making recommendations regarding care planning and funding arrangements.

Manage a complex caseload autonomously, prioritising workload effectively and delivering outcomes within agreed timescales and quality standards.

Provide specialist clinical advice and support to commissioners, providers and partner organisations on complex Continuing Healthcare and Mental Health cases.

Assess and review individuals with mental health needs, ensuring care arrangements are appropriate, effective and promote improved health and wellbeing outcomes.

Support the review and management of Section 117 and other Mental Health-related cases where required, providing clinical expertise and recommendations.

Work collaboratively with Integrated Care Boards (ICBs), Local Authorities, providers, service users, families and multidisciplinary teams to support effective assessment and care planning processes.

Develop and monitor measurable outcomes and recommendations to support ongoing review and quality assurance of commissioned services.

Contribute to service improvement, transformation and quality initiatives across multiple workstreams, demonstrating flexibility and adaptability to changing organisational priorities.

Ensure all assessments, reports and documentation are completed accurately and professionally, in accordance with regulatory, professional and information governance requirements.

Provide evidence and professional justification to support challenges, appeals, complaints and complex decision-making processes where required.

Build and maintain positive working relationships with key stakeholders, demonstrating credibility, professionalism and clinical leadership in all interactions.
EXPERIENCE
Essential:
- Registered Mental Health Nurse (RMN) with significant Continuing Healthcare (CHC) assessment and review experience.

Proven ability to manage complex cases, make sound clinical judgements and produce high quality evidence-based recommendations.

Experience working autonomously across multiple workstreams, delivering outcomes and managing key stakeholder relationships.
Desirable:

Experience of Section 117 Aftercare assessments and reviews.

Knowledge of the Mental Health Act, Mental Capacity Act and Care Act.

Experience in commissioning, service transformation or complex case review programmes.
SKILLS & COMPETENCIES
Essential
-
Strong clinical assessment, decision-making and report-writing skills, with the ability to make robust evidence-based recommendations.

Excellent knowledge of Continuing Healthcare (CHC) processes and the ability to manage complex cases autonomously.

Effective stakeholder management, communication and negotiation skills, with the ability to build positive relationships across health and social care systems.
Desirable
-
Knowledge of mental health legislation, including Section 117 Aftercare, the Mental Health Act and Mental Capacity Act.

Experience working across multiple programmes, workstreams or transformation projects.

Experience undertaking remote assessments and using digital case management systems.
QUALIFICATIONS & TRAINING
