Community Support Worker – Children & Young People (Staffordshire)
We are looking for compassionate, experienced Support Practitioners to join our growing team in Staffordshire.
This role offers a mix of flexible shorter shifts (2–6 hours) and longer wraparound support (days and waking nights 8-12 hours), supporting children and young people with high support needs in the community.
Who We Are
Care Bridge Specialists support individuals with autism, learning disabilities, mental health challenges, behaviours of concern, challenging social circumstances and complex needs, who are often considered too complex for standard services.
We are CQC registered to provide specialist, person-centred support to help people remain safely in the community, avoid hospital admission, and live meaningful, fulfilling lives.

About the Role
You will be part of a Practitioner team supporting children and young people with complex, high-acuity needs, including autism, learning disabilities and mental health comorbidities.
You will:
- Deliver person-centred support in line with care plans and risk assessments
- Use Positive Behaviour Support (PBS) and de-escalation approaches
- Support with daily living, routines, community access and independence
- Build trusting, therapeutic relationships with the individuals you support
- Work collaboratively with families, professionals and clinical teams
What We’re Looking For
Essential
- Business have a UK Driving License + Motor Business Insurance (you will be required to provide evidence of your business insurance and a copy of your vehicle MOT certificate).
- Up to 5+ years’ experience in health or social care (Band 4 equivalent)
- Experience supporting children or young people with complex needs
- Strong understanding of autism, learning disabilities and/or mental health comorbidities
- Strong communication and record-keeping skills, including clear written notes, handovers and incident reporting, with confidence using electronic care systems (e.g. Nourish) and contributing to MDT and professional meetings
- NVQ Level 2 (or Care Certificate minimum)
- Full UK driving licence
- Willingness to complete specialist training
- Experience supporting challenging behaviours and crisis presentations
- Training in PMVA or CPI Physical safety Interventions and Positive Behaviour Support
- Experience across ATU’s, hospitals, secure residential services and community settings
