About the Role
We are seeking a proactive and dedicated Care Coordinator to play a pivotal role in delivering high-quality, person-centred care to adults in their own homes and supported living settings.
Working closely with the Registered Manager and Operations Team, you will be responsible for coordinating care delivery, supporting staff, and ensuring services are Safe, Effective, Caring, Responsive and Well-Led in line with CQC standards.
This is a hybrid role combining office-based coordination, quality assurance and field-based responsibilities, including audits, spot checks and staff support. You will often be the first point of contact for carers, service users, families and healthcare professionals, ensuring continuity of care while promoting the organisation's services through professional face-to-face and telephone communication.
Key ResponsibilitiesCare Coordination & Service Delivery
- Effectively schedule and coordinate all care visits to ensure continuity, efficiency and reliability of care.
- Act as the central point of communication between service users, care teams, families and external professionals.
- Arrange, undertake and review care reviews, providing guidance and ongoing support to care staff.
- Promote the organisation's services and uphold its reputation as a high-quality care provider.
- Participate in the management on-call rota and provide operational support during emergencies when required.
Quality Assurance & Care Standards
- Conduct regular announced and unannounced spot checks and shadowing visits to monitor the quality of care delivery.
- Ensure care is delivered in accordance with individual care plans, risk assessments and CQC Fundamental Standards.
- Promote dignity, respect, compassion, equality, diversity and cultural sensitivity in all aspects of service delivery.
- Provide constructive coaching and feedback to care staff to encourage continuous improvement.
Compliance, Auditing & Safety
- Complete quality assurance audits, including Medication Management, Health & Safety, Infection Prevention & Control (IPC) and care documentation.
- Assess medication competencies, audit MAR charts and ensure the safe administration, storage and handling of medicines.
- Carry out health and safety assessments within service users' homes, including fire safety and environmental risk assessments.
- Promote compliance with Infection Prevention & Control guidance, appropriate PPE usage and safe working practices.
Documentation & Safeguarding
- Maintain accurate, timely and compliant records using digital care management systems, including CareBeans or equivalent software.
- Respond promptly to safeguarding concerns, escalating appropriately to the Registered Manager and relevant statutory agencies.
- Manage incidents, accidents, complaints and near misses, ensuring investigations, actions and lessons learned are documented and implemented.
Staff Support & Development
- Support the induction, mentoring and competency assessment of new and existing staff.
- Participate in supervision, probation reviews and appraisal processes, identifying learning and development needs.
- Act as a positive role model, promoting professionalism, accountability and organisational values.
Service Improvement
- Analyse audit findings and quality assurance outcomes to identify trends and contribute to continuous service improvement plans.
- Gather feedback from service users, relatives and professionals to improve service delivery.
- Work collaboratively with the management team to implement new policies, procedures and best practice.
- Liaise with Complex Care Nurses and multidisciplinary professionals to coordinate client-specific training, competency assessments and joint reviews.
Qualifications & ExperienceEssential
- Minimum of two years' experience within domiciliary care, supported living or community care services.
- Level 2 Diploma in Health & Social Care (or willingness to work towards Level 3).
- Strong understanding of CQC Regulations, the Single Assessment Framework, Health and Social Care legislation and Safeguarding Adults.
- Experience conducting audits, quality assurance checks or care assessments.
- Full UK driving licence and access to a vehicle.
- Excellent communication, organisational, observational and report-writing skills.
- Commitment to delivering person-centred care while promoting independence, dignity, equality and inclusion.
Desirable
- Level 3 Diploma in Health & Social Care or above.
- Previous experience in a Care Coordinator, Senior Carer or Supervisory role.
- Training in Medication Administration, First Aid and Safeguarding.
- Experience using digital care management systems such as CareBeans or similar.
- Leadership and Management qualification or currently working towards Level 4 or Level 5.

Employment Details
Job Type: Permanent
Work Location: Hybrid (Office, Community and Field Based)
Working Hours: Monday to Friday, 8:30am – 5:30pm, with participation in the on-call rota as required.
Annual Leave: 28 days per annum, inclusive of bank holidays.
