About the Role
We are looking for a compassionate, experienced, and motivated Senior Care Worker/Care Coordinator to join our growing team. This role combines the delivery of outstanding person-centred care with leadership responsibilities, supporting and guiding care staff to ensure the highest standards of service for our clients.
You will play a key role in promoting independence, dignity, and wellbeing while helping to coordinate care, monitor service quality, and support the development of our care team.
Key Responsibilities
Care Delivery
- Provide high-quality personal care, including assistance with washing, dressing, grooming, toileting, and other daily living activities.
- Support clients with mobility using appropriate equipment such as hoists, wheelchairs, or walking aids where required.
- Administer or assist with medication in accordance with company policies and procedures.
- Prepare meals, encourage adequate nutrition and hydration, and support clients with eating where necessary.
- Provide companionship, emotional support, and encourage participation in meaningful activities.
- Carry out light household duties to maintain a safe, clean, and comfortable home environment.
- Monitor clients' physical, emotional, and mental wellbeing, reporting any concerns promptly.
- Develop, implement, and regularly review person-centred care plans to ensure they reflect each individual's needs and preferences.
Care Coordination & Leadership
- Coordinate care services to ensure clients receive safe, effective, and responsive support.
- Supervise, mentor, and support Care Workers, acting as a positive role model within the team.
- Assist with the induction, training, and ongoing development of new and existing staff.
- Conduct spot checks, observations, and supervision sessions to monitor care quality and staff performance.
- Provide constructive feedback and support continuous improvement.
- Assist in the assessment of new clients and contribute to care planning and reviews.
- Lead the care team during shifts and provide guidance during emergencies or unexpected situations.
Communication & Compliance
- Build positive relationships with clients, families, advocates, and healthcare professionals.
- Work collaboratively with GPs, social workers, district nurses, and other multidisciplinary professionals.
- Attend care review meetings and contribute to service planning.
- Maintain accurate and confidential care records, reports, and documentation.
- Report safeguarding concerns promptly and follow safeguarding procedures.
- Ensure compliance with all relevant legislation, CQC standards, company policies, and health and safety requirements.
Travel Requirements
- A full UK driving licence and access to your own vehicle are essential, as travel between clients' homes is required.
- Be willing to accompany clients to appointments or community activities where appropriate
