The Frailty SDEC Clinical Lead is responsible for the operational and clinical leadership of the Frailty SDEC service, ensuring the delivery of safe, effective, person-centred care for older people with frailty. The postholder will provide visible clinical leadership, coordinate multidisciplinary working, support admission avoidance and timely discharge pathways, and ensure the service dleivers high quality care aligned with the Trust Integrated Cre System and National frailty standards
We require a motivated, compassionate and enthusiastic leader for our Frailty Same Day Emergency Care service. The service provides timely multidisciplinary assessment and intervention for people living with frailty, supporting admission avoidance, early supported discharge and safe transfer of care.
Proven leadership, excellent interpersonal skills and the ability to influence, support and motivate others are essential to this role.
- Provide a focused leadership role to manage staff and activities within Frailty SDEC.
- Act as a visible role model and maintain the highest standards of patient care.
- Lead and coordinate the nursing team within the service, demonstrating compassionate leadership and effective role modelling.
- Work collaboratively with the multidisciplinary team to deliver comprehensive assessment, treatment and discharge planning.
- Support admission avoidance and timely discharge using a Home First approach.
