Responsibilities
- Develop and implement personalized care plans tailored to each resident’s unique needs, goals, and circumstances.
- Support residents with daily activities, including personal care, household tasks, and social engagement.
- Foster open and respectful communication with residents, their families, and external service providers to ensure coordinated care.
- Monitor residents’ well-being regularly, identifying any concerns or changes that require additional support or intervention.
- Encourage independence by empowering residents to participate actively in their own care and community life.
- Maintain accurate records of support activities, progress notes, and incident reports using IT systems efficiently.
- Drive community engagement initiatives that promote social inclusion and positive interactions among residents.
Requirements
- Proven experience working with children or vulnerable adults in supported housing
- Strong communication skills with the ability to listen actively and convey information clearly.
- Ability to develop and follow detailed care plans tailored to individual needs.
- Valid driving license and willingness to use a vehicle for work-related travel when necessary.
- Competence in using IT systems for record-keeping, reporting, and communication purposes.
- Knowledge of safeguarding procedures, health & safety protocols, and confidentiality standards.
- Flexibility to work varied shifts including evenings, weekends, or on-call duties if required.

Pay: £25,420.00-£29,445.00 per year
