Working Hours Full-time, 37.5 hours per week
Salary £20– £25/hr (dependent on experience)
Job Summary:
The Mental Health Practitioner (MHP) will work as part of a Primary Care Network
(PCN) multidisciplinary team to deliver high-quality mental health support within
primary care settings.
The post holder will assess, support, and manage patients presenting with mild to
moderate mental health difficulties, providing evidence-based interventions, brief
therapeutic input, and appropriate onward referral to specialist services where
required. The role is designed to support GP practices by improving access to timely mental
health care and reducing pressure on secondary care services.
Key Responsibilities:
- Undertake holistic mental health assessments within a primary care setting
using evidence-based frameworks
- Provide brief interventions for mild to moderate mental health conditions
(e.g. anxiety, depression, stress-related disorders)
- Develop, implement, and review person-centred care plans in collaboration
with patients and the multidisciplinary team
- Deliver low-intensity psychological interventions, psychoeducation, and
guided self-help approaches
- Undertake risk assessment and management, escalating appropriately to
senior clinicians when required
- Signpost and refer patients to Talking Therapies (IAPT), secondary care, or
other services as appropriate
- Support GP practices in managing mental health caseloads within primary
care
- Maintain accurate, timely clinical documentation in line with professional
and organisational standards
- Participate in MDT meetings, case discussions, and service development
activities
- Work collaboratively with GPs, social prescribers, and other ARRS roles

- Promote recovery-focused, patient-centred care in line with NHS policies
and guidance
Essential Qualifications and Experience:
- Registered Mental Health Nurse (RMN) with valid NMC registration OR
equivalent qualified mental health professional (e.g. OT, PWP, social
worker)
- Minimum 2 years post-registration experience in mental health services
- Experience working with mild to moderate mental health conditions or
community mental health settings
- Strong communication and interpersonal skills
- Knowledge of mental health legislation, safeguarding, and risk assessment
- Ability to work independently within a primary care environment
Desirable
Qualification:
- Experience working in Primary Care / GP Surgery / PCN setting
- Experience in Talking Therapies (IAPT) or brief psychological interventions
- MAPA / PMVA training
- Experience in CAMHS, addictions, eating disorders, or older adult mental
health
- Experience using clinical systems such as EMIS
- Non-medical prescribing qualification (V300)

No Smoking Policy • The company has a strict No Smoking Policy as part of its responsibility for the
provision of health. The Post Holder will be required to work within the
framework of this policy. Smoking is not permitted anywhere within the
company buildings or at any location where services are provided.

Mandatory / Essential:
Registered Mental Health Nurse (RMN) qualification with a valid NMC PIN.
Minimum of 2-years post-registration experience in a mental health setting.
Strong communication and interpersonal skills.
Knowledge of current mental health legislation and best practices
