Community Wellbeing Worker

20 hours per week
OVERALL PURPOSE OF ROLE
The Wellbeing Worker will provide practical and emotional support to service users. The role focuses on improving wellbeing, reducing isolation, promoting independence, and helping individuals access appropriate services and opportunities within the community.
The successful candidate will build positive, professional relationships with service users and support them in achieving personal goals through person-centred support plans and be involved in group support, community engagement activities, and attendance at local events.
RESPONSIBILITIES
1. Provide one-to-one wellbeing support to service users by offering emotional support, encouragement, and practical guidance to improve confidence, independence, and resilience.
2. Manage referrals into the Community Wellbeing Project.
3. Carry out wellbeing assessments and contribute to support planning based on individual needs and goals.
4. Maintain accurate, confidential, and up-to-date records, case notes, and monitoring information.
5. Monitor progress and review outcomes with service users regularly.
6. Encourage participation in community events and activities to promote social inclusion and reduce isolation.
7. Support the delivery of workshops, wellbeing activities, outreach events, and community engagement sessions where required.
8. Attend community events, wellbeing activities, outreach sessions, and partnership meetings to support service users and promote the project.
9. Build positive relationships within the local community and develop links with partner organisations and local services.
10. Safeguard vulnerable adults and follow organisational safeguarding procedures at all times.
11. Attend team meetings, supervision sessions, and relevant training opportunities.
Requirements
1. Always maintain confidentiality.
2. Adhere to the charity’s policies and procedures and ensure that high standards of quality and good practice are maintained, developed and monitored.
3. Undertake training as appropriate and attend regular 1 -1 supervision sessions with the Community Wellbeing Manager
4. Demonstrate awareness/understanding of equal opportunities and other people’s behavioural,
physical, social and welfare needs.
5. Ensure that reasonable care is always taken for the health, safety and welfare of yourself and
other persons, and comply with the policies and procedures relating to health and safety.
6. Have a flexible approach.
7. Undertake any other duties of a similar level and responsibility as may be required from time to
time.
This role is funded by the Community Wellbeing Partnership initially for a 1-year period with potential to be extended.
