Duties
- Provide personalized patient and resident care, including assistance with daily activities such as bathing, dressing, grooming, and mobility support
- Administer medications accurately according to prescribed care plans while maintaining detailed documentation
- Support behavioral management strategies to assist individuals with dementia or other cognitive impairments
- Assist with meal preparation and feeding to ensure nutritional needs are met in a safe manner
- Conduct routine health checks and monitor vital signs to identify any changes in condition promptly
- Maintain accurate records of patient care activities, medication administration, and incident reports
- Support case management efforts by coordinating with healthcare professionals to develop and implement individualized care plans
- Engage in light housekeeping duties to uphold a clean and safe environment for residents and clients
- Drive or accompany clients on errands or appointments as needed, ensuring safe transportation and adherence to schedules
Experience
- Prior experience working in nursing homes, care homes, assisted living facilities, or providing home care is highly preferred
- Knowledge of medical terminology and basic nursing skills is essential for effective patient support
- Experience with Alzheimer’s care, dementia management, hospice care, or behavioral management enhances your ability to deliver specialized support
- Certification in first aid is required; additional certifications such as medication administration or caregiving training are advantageous
- Demonstrated ability to lift heavy objects safely and assist with mobility tasks while adhering to safety protocols
- Familiarity with creating and following individualized care plans and case management processes
- Valid driver’s license and driving experience are beneficial for roles involving transportation responsibilities
