Position Overview:
We are excited to announce a wonderful opportunity for a Care Coordinator to join our friendly team of experienced professionals at our Doncaster branch. In this role, you will play a crucial part in providing premium customer service, coordinating and scheduling our care packages to ensure our clients receive continuity of care in their own homes.
Job Description:
· Main Function of the Job:
· Provide leadership to the support functions of the service.
· Supervise the support services within the organization in accordance with agreed standards.
· Maintain support skills and undertake necessary training and development to keep practices up to date.
Main Duties:
· Adhere to and implement all company policies and procedures, including Equality and Diversity, to achieve organizational aims.
· Assist the branch manager in all aspects of care management within the designated area.
· Address issues between Care Workers and Service Users effectively on a daily basis.
· Act professionally as a representative of the company to support its good reputation.
· Prepare schedules considering travel time, holidays, training, and last-minute cancellations.
· Coordinate Care Staff and resources while monitoring individual care packages to provide a person-centred service.
· Ensure the efficient delivery of services to all Service Users.
· Liaise with Social Services and relevant social care agencies to provide effective support to clients.
· Compile and maintain accurate records, ensuring they remain up to date daily.
· Ensure the service delivered meets the requirements of the Service User.
· Comply with Health and Safety legislation, informing the Registered Manager of any incidents.
· Be on-call evenings and weekends, managing the on-call phone.
· Provide cover for any outstanding care calls during office hours and on-call shifts.
· Maintain a professional relationship with peers, supporting a cohesive team environment.
· Uphold confidentiality in line with the company’s Confidentiality Policy.
· Monitor the service in the field to meet quality targets and contractual requirements and keep the Registered Manager informed of the outcomes and identified issues.
· Conduct comprehensive assessments for potential new Service Users before agreeing to packages.
· Support care staff by holding team meetings as discussed with the Registered Manager.
· Produce reports and maintain accurate records as requested by the Registered Manager.
· Undertake other relevant tasks as assigned.
· Allocate care assistants with the right skills and experience to ensure quality care and support.
· Provide exceptional service to care assistants, clients, and other professionals.
Essential Criteria:
· Experience in a line manager role within a healthcare or relevant environment.
· Proficient in IT systems, including Microsoft Office, Care Planner, or CRM software, with the ability to learn new technologies.
· Highly resilient, positive, and possess excellent communication skills.
· Strong organizational and prioritization skills, able to work calmly and accurately under pressure.
· Team player with strong interpersonal skills, capable of building rapport quickly.
· Excellent attention to detail with the ability to multi-task.
· Logical and analytical thinking, with the ability to work independently and meet deadlines.
When You’ll Be Working:
Monday to Friday, 9 AM to 5:00 PM
Participation in an on-call rota of approximately 1 week in 7
Qualifications:
· NVQ Level 2/3 in Health & Social Care (essential) or working towards it.

Experience:

Licence/Certification:
Full UK Driving Licence (required) with at least 1 year of experience.
Work Authorization:
