The role
- Hours: 37.5 hrs/week (Tue–Sat or Sun–Thu)
- Salary: From £24,784.50 (rising to £31,393.70 with sleep-in shifts after induction)
- When you are signed off as Support Navigator your salary will be increased.
You'll support individuals in the community to reach their personal goals, including:
- Creating and reviewing support, task and risk plans
- Tracking progress against personal goals and KPIs
- Building relationships with healthcare professionals and families
- Helping people access and enjoy their local community
- Managing individual and communal budgets
- Raising safeguarding referrals when needed
- Supporting with day-to-day home upkeep
