As a Health Team Coordinator, you will play a key role in ensuring the smooth operation of AKG Health's innovative, health-led approach to supporting individuals with complex barriers to progression. Working as part of a multidisciplinary team, you will coordinate participant activity, facilitate service delivery and help ensure participants receive a seamless, person-centred experience throughout their journey. Your contribution will enable Health and Employment Specialists to maximise participant-facing time whilst maintaining high standards of quality, engagement and contractual compliance. Participant and Team Support
- Coordinate the day-to-day operational support required by the Health and Employment Specialist team, ensuring participants receive a smooth and coordinated service throughout their journey. Act as a friendly and responsive point of contact for participants, supporting appointment arrangements, responding to routine enquiries and helping to maintain participant engagement between scheduled interventions. Where appropriate, contact participants who have missed appointments to encourage re-engagement and identify any practical barriers to attendance.
Service Coordination
- Support the efficient operation of the Confident Futures service by coordinating participant allocations, scheduling appointments and ensuring key contractual milestones are met. Work closely with the Service Delivery Manager and Health and Employment Specialists to maintain well-managed caseloads, monitor diary capacity and facilitate changes to appointments when required. Assist with the coordination of Warm Handovers and ensure participants move smoothly through each stage of the programme.
Operational Administration
- Maintain accurate participant records within the case management system, ensuring documentation is complete, up to date and compliant with contractual requirements. Support the collection of participant feedback, outcome measures and management information, assisting the wider team with quality assurance, reporting and audit activity to ensure the service operates efficiently.
Group and Service Facilitation
- Provide practical support for the delivery of virtual wellbeing sessions, webinars and community-based activities by coordinating invitations, preparing resources, booking venues where required and managing attendance. Work alongside Health and Employment Specialists and the Community Partnership Connector to ensure activities are well organised and participants receive a positive experience.
Quality, Safeguarding and Continuous Improvement
- Maintain high standards of confidentiality, information governance and data quality at all times. Escalate safeguarding or participant welfare concerns promptly in line with organisational procedures and contribute to a culture of continuous improvement by identifying opportunities to enhance service delivery, participant experience and operational efficiency.
