Head of Age Well
A leading Local Authority in Southwark is seeking an experienced Head of Age Well to join its commissioning team.
This is a key leadership role, jointly funded by the Local Authority and wider health system, with responsibility for commissioning services for older and/or frail residents, including those with physical disabilities.
The post holder will lead a commissioning team and work collaboratively across the Council, NHS, voluntary sector and wider health and care system to develop and deliver programmes focused on improving outcomes for residents across Southwark.
Key Responsibilities
- Lead and contribute to complex commissioning programmes across health and social care.
- Develop strong relationships with senior stakeholders across the Local Authority, NHS, voluntary sector and commissioned providers.
- Support the professional development of the commissioning team.
- Review existing commissioning arrangements and services to identify opportunities for improvement.
- Support high-quality commissioning with a focus on tackling inequalities, improving outcomes and generating value for Southwark residents.
- Work closely with Public Health Consultants, Heads of Programme and Policy Officers to develop and implement commissioning plans.
- Contribute to key health and care priorities across Southwark and the wider health system.
- Lead and coordinate programmes focused on older and/or frail residents and those with physical disabilities.
