JOB TITLE: Support North Northants: Community Link Worker
HOURS: 37 hours per week
Contract Length: to June 2027 (with a view to extend)
LOCATION BASE: Covering North Northants area (either Kettering & Corby, or Wellingborough & East Northants). Office base in Wellingborough.
SALARY: £28007.00 per annum (pro rata)

Support Northamptonshire, in partnership with Support North Northants (SNN), North Northamptonshire Council, Accommodation Concern, Citizens Advice North Northamptonshire and Community Law Service, has secured funding through the Crisis Resilience Fund (CRF) to expand support for people experiencing financial hardship and crisis across our communities.
As part of this exciting initiative, you will work alongside local partners in neighbourhoods across North Northamptonshire, helping people access the right support at the right time.
You'll support individuals to navigate complex systems, connect with financial assistance and community resources, and empower them to overcome challenges before they escalate.

SUMMARY OF POSITION
As a Community Link Worker, you will be at the heart of helping people access the support they need to improve their lives and wellbeing. You will often be the first point of contact for individuals and partner organisations, building trusted relationships and providing personalised support to people facing a wide range of challenges.
More than simply signposting or making referrals, you will work alongside individuals to understand their goals, develop tailored support plans, and guide them through every stage of their journey. You will empower people to overcome barriers, navigate services, and access the right help at the right time.
Working with people who may have multiple and complex needs, you will collaborate with a wide range of community, voluntary, health, and statutory partners to ensure individuals receive coordinated, person-centred support and achieve the best possible outcomes.

The ideal candidate will:
Main Duties:
1. Provide direct person-centred, outcomes focussed support for people in the form of key working.
2. Develop outcomes focussed support plans and provide practical support as required.
3. Remain the key support worker for individuals until all positive outcomes have been achieved.
4. Work with a range of partners as if it was one team.
5. Update and maintain records and monitoring, ensuring accurate data is kept.
6. Contribute to the continual improvement of service effectiveness.
7. Identify any need for advocacy to support for people as required.
8. Identify any safeguarding concerns and take appropriate actions.
9. Have local knowledge of services and how to mobilise these for service users, including local gaps to help inform our strategic thinking.
10. Identify risks and ensure appropriate action is taken to mitigate risks.
11. Raise the profile and awareness of Support North Northants
12. Provide administrative support to the Service User Forum.
13. To be a flexible support within the team, providing support where it is needed.
PERSONAL SPECIFICATION
Knowledge and Experience
1. 1-2 years’ experience of working as a Support Worker in a health and social care setting or similar field of work (ideally L3 H & SC Qualification or equivalent)
2. 1-2 years’ experience of working with local communities and community development or in social care area of work
3. Experience of person-centred, holistic and outcomes focussed practice desired
4. Experience of supporting the most vulnerable and ‘not giving up on people’.
5. A record of putting Equality, Diversity and Inclusion into practice and an ability to demonstrate how you have challenged all form of discrimination
6. Experience of reporting to senior management teams
