Position: Placement & Brokerage Officer
Location: 2 Alice Square, Birkenhead, Merseyside, CH412YA
Start Date: As soon as possible (Immediate start required)
Contract Duration: 3+ Months (2 days remote, 3 days in the office)
Working Hours: 36 hours per week
Pay Rate: PAYE: £14.28 per hour
Job Responsibilities
- Arrange and record care packages accurately using relevant case management systems, ensuring compliance with organisational standards
- Coordinate care arrangements for individuals with assessed needs, ensuring timely access to appropriate support
- Manage daily demand for care and support, prioritising urgent cases such as hospital discharges
- Work with care providers to identify availability, monitor capacity and support effective placement decisions
- Maintain accurate records of provider capacity, market information and care package availability
- Carry out quality checks on brokerage referrals to identify and resolve errors or missing information
- Provide advice and guidance to social care and health professionals on care arrangements and available options
- Liaise and negotiate with care providers, social workers, health professionals and service users to secure suitable care packages
- Support out-of-area placements and ensure arrangements meet service requirements and policies
- Build and maintain positive relationships with internal teams, external providers and partner organisations
- Support the commissioning and brokerage of services in line with contractual requirements and agreed rates
- Analyse capacity and demand information to support effective decision-making
- Identify and escalate risks, market pressures, service issues and areas for improvement
- Maintain accurate case records and ensure compliance with GDPR, confidentiality, health and safety and organisational policies
- Manage competing priorities, meet deadlines and contribute to continuous improvement within the service
Requirements
Essential Knowledge, Skills and Experience
- Experience working within an Adult Social Care environment
- Knowledge of care arrangements, brokerage processes and supporting individuals with assessed care needs
- Strong verbal and written communication skills with the ability to communicate complex information clearly
- Ability to build effective relationships with service users, care providers, professionals and partner organisations
- Ability to manage competing priorities and work effectively in a fast-paced environment
- Good organisational skills with strong attention to detail and ability to maintain accurate records
- Confident using digital systems, case management systems and Microsoft Office applications
- Ability to analyse, interpret and present information and data
- Ability to work collaboratively as part of a team and support service delivery
- Understanding of confidentiality, equality, GDPR and professional standards
- Ability to work flexibly and adapt to changing service requirements
Desirable Knowledge, Skills and Experience
- Experience delivering health and social care services or providing care-related advice
- Experience using systems such as Liquidlogic, E-Brokerage or Power BI
- Experience working with care providers, commissioning teams or placement services
Qualifications
- GCSE level education or equivalent, including Maths and English
- Level 2 or above qualification in IT/Computer Literacy (desirable)
