Care Coordinator – Job Description
We are seeking a dedicated, organised, and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in coordinating and managing client care plans, ensuring effective communication between clients, healthcare professionals, and support services.

The successful candidate will support the organisation in maintaining high-quality care delivery and achieving a CQC Good rating.
The ideal candidate must have a minimum of 5 years’ experience working within the UK health and social care sector, including experience within CQC-regulated services.
Key Responsibilities
- Coordinate and schedule client appointments, care visits, treatments, and follow-up care.
- Develop, implement, and monitor person-centred care plans to ensure they meet the individual needs of clients.
- Maintain accurate, up-to-date care records and documentation in line with organisational policies and regulatory requirements.
- Act as a key point of contact between clients, families, healthcare professionals, and support staff to ensure clear and effective communication.
- Support the day-to-day coordination of care services to ensure continuity, safety, and quality of care.
- Monitor client progress and update care plans where required.
- Conduct occasional site visits, client reviews, and staff support visits where necessary.
- Assist with administrative duties including data entry, documentation, filing, and correspondence.
- Coordinate resources and support services tailored to individual client needs.
- Ensure compliance with Care Quality Commission (CQC) regulations, safeguarding policies, and confidentiality standards.
- Support internal audits, quality monitoring, and service improvement initiatives to help the organisation achieve and maintain a CQC Good rating.
Qualifications and Experience
- Minimum of 5 years’ experience working within the UK health and social care sector.
- Experience working within a CQC-regulated service and strong understanding of CQC Fundamental Standards.
- Previous care coordination, administrative, or office experience within a care setting is highly desirable.
- Knowledge of care planning, safeguarding procedures, and quality assurance in health and social care.
- Excellent organisational and time-management skills.
- Strong written and verbal communication skills.
- Compassionate, professional, and committed to person-centred care.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in standard office software programmes (e.g., MS Office).
- Must hold a valid UK driving licence and have access to a vehicle, as travel may be required to visit clients and services.
- Strong attention to detail and high standards of record keeping.

Pay: £25,500.00-£30,000.00 per year
