The Role
As a Dementia Practitioner, you will play a vital role in delivering exceptional care to residents at Willows Edge Care Home. Reporting to the Deputy Manager, you will be responsible for:

Implementing and maintaining high standards of dementia care in accordance with best practice guidelines.
- Providing direct care and support to residents, ensuring their physical, emotional, and social needs are met.
- Monitoring and evaluating residents' progress, making appropriate adjustments to care plans as necessary.
- Collaborating with multidisciplinary teams, including healthcare professionals, to ensure holistic care delivery.
- Advising and supporting families and carers, providing guidance on dementia-related issues.
- Contributing to the continuous improvement of care services through participation in audits and quality assurance processes.
Please note for this role requires working every other weekend
The Ideal Candidate
The ideal candidate will possess the following skills and attributes:

Extensive knowledge and understanding of dementia and its impact on individuals and their families.
- Proven experience in providing high-quality care to individuals with dementia, ideally within a residential or nursing home setting.
- Strong communication and interpersonal skills, with the ability to build positive relationships with residents, families, and colleagues.
- Ability to work effectively within a multidisciplinary team, demonstrating collaboration and cooperation.
- Commitment to continuous professional development and staying up-to-date with current best practices in dementia care.
