Solsken Ltd is a specialist complex care provider delivering bespoke packages of care for children and adults across the UK. We support individuals with complex health needs to live safely and independently within their own homes and communities.
As our services continue to grow, we are seeking an experienced and compassionate Registered General Nurse (RGN) to join our Clinical Team as a Clinical Case Manager.
This is an exciting opportunity for a nurse who enjoys working autonomously, building relationships with families and professionals, developing services and ensuring consistently high standards of care.
About the Role
As a Clinical Case Manager, you will provide clinical oversight and leadership across a range of adult and paediatric complex care packages.
Working closely with the Clinical Lead, families, multidisciplinary professionals and support teams, you will ensure the delivery of safe, effective and person-centred care while supporting the ongoing development of our services.
This is a varied role combining community visits, clinical assessments, staff development, quality assurance and clinical governance responsibilities.
Key Responsibilities
- Provide clinical oversight and management of complex care packages.
- Complete assessments, care reviews and community visits.
- Develop, implement and evaluate person-centred care plans and risk assessments.
- Deliver clinical training and practical skills assessments to support staff.
- Complete clinical supervision and competency assessments.
- Conduct audits and quality assurance reviews.
- Liaise with families, commissioners, case managers and multidisciplinary teams.
- Investigate incidents and contribute to continuous quality improvement.
- Maintain high-quality clinical documentation.
- Support the organisation's clinical governance and regulatory compliance processes.
Essential Requirements
- Registered General Nurse (RGN) with an active NMC PIN.
- Minimum 3 years post-registration experience.
- Experience within complex care, community nursing or continuing healthcare environments.
- Experience supporting adults and/or children with complex healthcare needs.
- Strong clinical assessment and risk management skills.
- Excellent communication and relationship-building skills.
- Ability to work independently and manage a varied caseload.
- Full UK driving licence and access to a vehicle.
Desirable Experience
Experience supporting individuals with:
- Tracheostomy care.
- Enteral feeding (PEG/PEJ).
- Ventilation support.
- Complex medication regimes.
- Epilepsy and seizure management.
- Neurological conditions.
- Paediatric complex care.
- Continuing Healthcare (CHC) funded packages.
