Responsibilities
- Deliver personalized patient care by assessing, planning, and implementing diabetes management strategies
- Educate patients on blood sugar monitoring, insulin administration, dietary choices, and lifestyle modifications
- Administer medications safely and accurately according to prescribed protocols
- Monitor patient health status through regular check-ups and blood glucose testing
- Collaborate with multidisciplinary teams to develop and adjust treatment plans based on patient progress
- Maintain detailed and accurate medical records in compliance with healthcare regulations
- Provide emotional support and counselling to motivate patients toward healthier habits and self-care practices
Qualifications
- Proven experience in patient care within a clinical or community healthcare setting
- Strong knowledge of human anatomy, physiology, and the pathophysiology of diabetes
- Proficiency in medication administration, including insulin injections and other diabetic medications
- Excellent communication skills to educate and motivate patients effectively
- Ability to work independently while collaborating seamlessly with healthcare teams
- Relevant nursing qualification (NMC Registration)
