ABOUT THE ROLE
MAK Community Care is seeking dedicated and compassionate Mental Health & Learning Disabilities Support Team Members who drive and hold Business Insurance. You will support adults within their own homes and shared accommodation, helping them live independently and confidently.
This role is ideal for someone who is person‑centred, reliable, confident, and able to work with minimal supervision.
MAIN RESPONSIBILITIES, TASKS & DUTIES
Support Delivery
- Follow day‑to‑day responsibilities as outlined in individual care/support plans, ensuring person‑centred practice, accurate reporting, and record keeping.
- Provide hands‑on support to individuals in line with their support plans.
- Participate in and oversee the delivery of services to the people we support.
- Provide effective leadership as a team member through rota implementation, planned sessions, hands‑on support, and mentoring.
- Maintain accurate administrative records and ensure they are updated and archived appropriately.
- Lead by example, motivate colleagues, and support team development.
- Ensure environmental hygiene standards are maintained across all building bases before, during, and after sessions.
Support
- Actively participate in and oversee care delivery to ensure physical, social, emotional, and intellectual needs are met while promoting dignity, choice, and independence.
- Monitor health needs, ensure access to healthcare services, and maintain accurate health‑related documentation.
- Promote independence through appropriate risk management and up‑to‑date record keeping.
- Encourage participation in meaningful activities that stimulate engagement and personal development.
- Assist team members in creating session plans that introduce new experiences and prevent stagnation.
- Work closely with individuals, families, carers, and staff, offering professional and emotional support.

General Duties
- Follow all MAK Community Care policies, procedures, and relevant regulations.
- Maintain accurate administrative records and ensure proper archiving.
- Undertake personal care tasks when required, respecting dignity and independence.
- Follow procedures for accidents, emergencies, fire, missing persons, and medication errors.
- Liaise with managers to ensure effective organisation and delivery of sessions.
- Ensure buildings remain secure and individuals remain safe at all times.
- Maintain agreed staffing levels in liaison with the Registered Manager.
- Report repairs and maintenance needs promptly.
- Work flexibly to meet service needs.
- Be approachable and supportive to staff and managers.
- Attend staff meetings, supervisions, and appraisals as required.
- Attend training and development sessions as agreed.
- Contribute to care/support plans and risk assessments.
- Communicate relevant information effectively to staff.
- Carry out any reasonable duties requested by the Manager.
- Comply with Health and Safety at Work Act requirements.

- Report safeguarding concerns to the Registered Manager and follow procedures.
- Maintain confidentiality at all times.
- Ensure hygiene standards are upheld across all properties in line with tenancy agreements.
REQUIREMENTS
- Must have a vehicle and Business Insurance
- Must be willing to transport service users to appointments, activities, and social outings
- Ability to travel reliably to and from work
- Fluent in English
- Must have the right to work in the UK
- Enhanced Adult DBS Check (required for all employees and volunteers)
