About the Role
Are you an experienced Registered Nurse (Paediatric, Mental Health, General, or Learning Disability) or Allied Health Professional with expertise in managing complex care across the lifespan? Looking for a flexible part-time role that offers true work-life balance?
We are seeking a *Complex Care Clinical Lead* to join our team on a flexible 3-day schedule (22 hours per week) across County Durham.
In this key leadership role, you will report directly to our Home Care Manager, who oversees business operations. Serving as the primary clinical lead within our multidisciplinary structure, you will work collaboratively to lead, guide, and oversee the clinical delivery of high-quality, person-centred care plans for children, young people, and adults presenting with complex medical conditions, physical disabilities, and co-occurring mental health needs.
Key Responsibilities
* *Clinical Accountability & Leadership:* Report directly to the Home Care Manager to oversee, guide, and mentor nurses and support workers, ensuring evidence-based clinical practices and regulatory compliance across all care packages.
* *Collaborative Operations:* Work hand-in-hand with the Home Care Manager and management team to align clinical care delivery with operational strategy and service standards.
* *Lifespan Holistic Assessment:* Conduct comprehensive clinical, developmental, mental health, and risk assessments for children, young people, and adults with complex health needs.
* *Care Coordination:* Formulate and manage robust, individualised care plans in collaboration with Integrated Care Boards (ICBs), NHS trusts, Child and Adolescent Mental Health Services (CAMHS), Adult CMHTs, social services, schools, and family care networks.
* *Transition Management:* Lead and support smooth clinical transitions from paediatric services to adult complex care networks for young people reaching adulthood.
* *Crisis Management & Safeguarding:* Oversee clinical crisis escalation protocols and manage complex behavioral, psychological, and medical risks while upholding robust child and adult safeguarding standards (Level 3).
* *Quality & Governance:* Audit care delivery, oversee clinical competency assessments (e.g., ventilation, tracheostomies, enteral feeding), and ensure full compliance with healthcare regulations alongside the Home Care Manager.
Requirements & Qualifications
Essential:
* *Professional Registration:* Active registration with the NMC (RSCN/Paediatric, RMN, RGN, or RNLD) or HCPC (Occupational Therapist or Social Worker).
* *Clinical Experience:* Minimum 3–5 years of post-registration experience directly supporting children, young people, and adults with complex medical needs and mental health conditions.
* *Specialist Knowledge:*
* Proven expertise in complex clinical interventions (e.g., enteral feeding, respiratory/tracheostomy care, seizure management, or severe physical/neurological disability).
* Demonstrated background in mental health management, emotional support, and neurodiversity across age groups (e.g., trauma-informed care, risk mitigation, Autism/ADHD support).
* *Safeguarding Knowledge:* In-depth working knowledge of both Level 3 Child Safeguarding and Adult Safeguarding practices.
* *Mobility:* Full UK driver’s license with access to a vehicle for community visits across County Durham.
Desirable:
* Dual registration (e.g., RSCN/RMN or RGN/RMN) or postgraduate training in paediatric complex care or mental health.
* Membership with relevant professional nursing or clinical bodies.
* Direct experience managing transition pathways from children’s to adult health services.
