Job Summary
We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will be responsible for organising and managing care plans, coordinating communication between clients and healthcare professionals, and ensuring the delivery of high-quality support services. Duties
- Organise and maintain comprehensive care plans tailored to individual client needs.
- Coordinate appointments, services, and communication between clients, families, and healthcare providers.
- Monitor and review care delivery to ensure adherence to established standards and client satisfaction.
- Maintain accurate records of all interactions, updates, and care documentation using office systems.
- Respond promptly to client enquiries via phone or email, providing clear information and support.
- Collaborate with team members to ensure seamless service delivery and continuous improvement.
- Assist in scheduling staff shifts and managing logistical arrangements for care services.
- Support clients in navigating available resources and community services as needed.
Skills
- Proven experience in an office environment with strong organisational skills.
- Excellent communication skills, both written and verbal, with a compassionate approach towards clients.
- Ability to manage multiple priorities efficiently whilst maintaining attention to detail.
- Proficient in using office software such as Microsoft Office or similar programmes.
- Ability to work independently as well as part of a multidisciplinary team.
- Empathy, patience, and a genuine desire to support individuals in need of care.

Pay: £27,000.00-£32,000.00 per year
