This is an exciting opportunity for an experienced Physiotherapist to join our Community team in Stewartry. Working within a multidisciplinary setting, you will support people living with frailty to maximise independence and wellbeing, enable early discharge, and help prevent avoidable hospital admissions.
Home Teams are integrated, multidisciplinary groups that work together to support people to achieve their goals and maintain independence at home. They bring together staff from Community Health and Social Care, including Community Nursing, Allied Health Professionals, Patient Flow, Reablement, Adult Social Work, Public Health Improvement and Business Support, creating one coordinated team around the individual.

We are seeking an experienced clinician to work autonomously across a wide range of health and social care settings. You will deliver specialist assessment, treatment and ongoing management for adults with diverse and often complex needs, contributing to coordinated, personcentred care alongside partners in health, education and social care.
You will manage a complex caseload using advanced clinical skills, supporting safe transitions from hospital to home and reducing the risk of readmission through effective communitybased intervention. The role also includes case coordination, health promotion, audit activity and providing expert advice on professional interventions.
You will supervise and support less experienced colleagues, including support workers and students, and offer guidance to the wider interdisciplinary team and external agencies. Working independently, you will assess, plan, implement and evaluate care to ensure highquality, safe and effective outcomes across both community settings and the acute interface. Part of your role will be to provide Physiotherapy leadership across the Home Teams, working in collaboration with the Home Team Leads within the new collaborative leadership model. In this capacity, you will work alongside other professional leads to support integrated, highquality service delivery.
