The Tobacco Dependency Advisor will support the delivery of a Trust-wide Tobacco Dependency Service, helping patients to quit smoking and improve their overall health.
The postholder will provide evidence-based behavioural support and treatment to patients identified as smokers, primarily within acute hospital settings, ensuring all eligible patients are offered appropriate interventions in line with national guidance.
Working as part of a multidisciplinary team, the role involves engaging patients in their quit journey, delivering personalised care plans, and supporting the implementation of effective tobacco dependency pathways across the Trust.
Deliver smoking cessation interventions, including behavioural support and advice on pharmacotherapy (e.g. nicotine replacement therapy).
Assess patients, develop personalised quit plans, and support individuals through their quit journey in both inpatient and community settings.
Provide one-to-one and group support sessions, including follow-up care (e.g. post-discharge telephone support).
Identify and engage patients who smoke, offering evidence-based advice and promoting harm reduction approaches.
Support the implementation of the Tobacco Dependency pathway and Ottawa Model of Care within the Trust.
Work collaboratively with clinical teams and community services to ensure continuity of care and effective referral pathways.
Monitor patient progress, record outcomes, and contribute to reporting requirements and service evaluation.
Promote smoke-free policies and contribute to health promotion campaigns (e.g. Stoptober).
Maintain accurate patient records and ensure compliance with data protection and governance standards.
Contribute to service development, audits, and quality improvement initiatives.

This will not constitute a formal merger of the two organisations, but will bring the Trusts together under a single Board and Executive Leadership Team, with the goal of improving the care that is provided to patients both in the community and in hospitals across Lincolnshire. Both Trusts will retain their separate statutory names and legal obligations.
The Group will be known as Lincolnshire Community and Hospitals NHS Group (LCHG).
