Company Description Active Minds is a welcoming community day centre supporting people aged 50 and over who are living with dementia, social isolation, anxiety, and depression. We provide person-centred care and engaging activities that promote wellbeing, independence, and meaningful social connections. Our mission is to create a safe, inclusive environment where every individual and their carer feels valued, supported, and empowered to live well.
Role Description As part of our supportive team, you will:
- Build trusting and respectful relationships with centre members, ensuring everyone is treated with dignity, compassion, and empathy.
- Provide personal care where required, including assistance with toileting, mobility, and eating, while always promoting independence and preserving privacy.
- Support and encourage participation in a wide range of engaging activities, including:
- Music therapy
- Exercise and wellbeing sessions
- Arts and crafts
- Baking
- Gardening
- Games and quizzes
- Cultural celebrations
- Community events and social activities
- Help reduce loneliness and social isolation by creating a welcoming, inclusive, and stimulating environment.
- Assist with meal preparation and service, encouraging good nutrition and hydration, and supporting members who require assistance during mealtimes.
- Support members safely during outings and day trips, including transport where applicable.
- Observe and record changes in wellbeing, reporting any concerns promptly to the management team or relevant professionals.
- Work closely with families, carers, healthcare professionals, and colleagues to provide holistic, person-centred support.
- Assist with cleaning and sanitisation duties to ensure the centre remains safe, welcoming, and compliant with health and safety standards.
- Uphold safeguarding responsibilities and consistently demonstrate the values of Active Minds.
