About us.
Five Oaks Care was established 25 years ago and is a residential care provider based on the fringes of Abergavenny and have four homes in the local area supporting young adults who have:
- Learning disabilities and associated conditions,
- Autism
- Mobility issues
We have a full-time Support Worker vacancy in our homes in Llanellen, Abergavenny, where our team provide help and support to 6 young adults across 2 homes on the same site. Our support worker teams are fully involved in every aspect of an individual’s life, which includes activities in the homes or community, day trips, going to the cinema or a meal out, and short break holidays to name a few. They also support with meal preparation, personal care, and medication administration, following person centred care plans.
We strive to make sure that everyone in our care will have the opportunity to experience new things and enjoy life to the full, but to do this we need your help!
Our day shift starts at 7.30am and ends at 10pm and a full-time contract would see you working 3 separate days across a 7 day week, so plenty of time off away from work or plenty of opportunity to work overtime, if it's available.
This role does require sleep-ins and on calls ( both paid at an additional rate)

Qualifications
- Strong communication skills
- Experience working with adults in a supportive role
- Valid driver's licence and ability to drive
- Knowledge of adult care practices and care plans
- Proficient in using IT tools and software for documentation and communication
- Ability to develop and implement care plans tailored to individual needs
