Job Overview
We are seeking a dedicated and organised Care Co-ordinator to join our team. The Care Co-ordinator will be responsible for developing and implementing personalised care plans, and maintaining excellent relationships with residents, families, and healthcare professionals. Duties
- Develop, review, and update personalised care plans in collaboration with residents, families, and healthcare professionals
- Review admission documentation to ensure smooth admissions of both temporary and perminent residents
- Facilitate effective communication between residents, families, staff, and external healthcare providers
- Ensure compliance with health and safety regulations within the care environment
- Drive organisational initiatives aimed at improving resident wellbeing and service quality
Experience
- Proven supervisory experience within a care home or senior care setting
- Experience in developing and managing care plans tailored to individual needs
- Excellent communication skills, both verbal and written, with the ability to liaise confidently with diverse stakeholders
- Competent IT skills for maintaining accurate records and managing schedules efficiently
