A leading specialist care provider is looking to recruit an experienced Complex Assessments Manager to join its growing team, supporting services across Travelling across Lancashire/Yorkshire.
This is a newly created and strategically important role, working closely with operational teams, commissioners, Local Authorities, NHS partners and multidisciplinary professionals to assess and mobilise complex referrals and develop sustainable, person-centred community support.

Working closely with operational and clinical teams, you will take the lead on complex referrals, assessments and transitions across Learning Disabilities, Autism, Mental Health and Complex Care services.
Your responsibilities will include:
- Assessing complex referrals and determining suitability, viability and sustainability
- Supporting hospital discharge, admission avoidance and community transitions
- Building strong relationships with commissioners, Local Authorities, ICBs, NHS Trusts and care managers
- Coordinating multidisciplinary assessments and specialist input
- Developing bespoke, community-based support packages
- Completing robust risk assessments and supporting safeguarding processes
To be successful in this role, you will have:
- A minimum of 3 years' experience in a senior practitioner, assessment, referrals, admissions, commissioning, operational or clinical role within Health & Social Care
- Strong experience supporting individuals with Learning Disabilities, Autism, Mental Health, Complex Care needs and Behaviours that Challenge
- Proven experience managing complex referrals, assessments, admissions, transitions and placement planning
- Strong assessment, analytical, risk management and report-writing skills
- Experience managing safeguarding concerns, restrictive practices and placement breakdowns
- Experience working with multidisciplinary teams, commissioners, Local Authorities, NHS Trusts and ICBs
- Excellent communication, organisation and relationship-building skills
- A minimum Level 3 Health & Social Care qualification
- A relevant professional qualification or equivalent experience in areas such as Nursing, Social Work, Psychology, Occupational Therapy, PBS, Learning Disabilities or Mental Health
- Full UK driving licence and access to a vehicle
Knowledge and understanding of the Mental Capacity Act, DoLS/LPS, Care Act, safeguarding, Positive Behaviour Support and community transition pathways would be highly beneficial.
Experience of Transforming Care, CTR/CETR, Dynamic Support Registers, service mobilisation, pathway development or commissioner engagement would also be advantageous.
