Indiana Healthcare Services is looking for an organised, motivated and experienced Care Coordinator to join our growing team in Truro.
Indiana Healthcare Services is a growing supported living and enabling care provider, supporting people across Cornwall to live as independently as possible within their own homes and communities.
Our services are focused on providing person-centred, enabling support that promotes independence, choice, dignity and meaningful outcomes. We support a wide range of individuals with different Mental Health and Learning Disabilities needs, working alongside service users, families, social workers, healthcare professionals and other partner organisations to ensure people receive the right support at the right time.
Our services include supported living, community-based support, domiciliary care and specialist care packages. We work across Cornwall, with our teams supporting people in their own homes and within their local communities.

As Care Coordinator, you will play a key role in ensuring our community and supported living services operate safely, effectively and in line with the needs of the people we support. You will work closely with support workers, senior staff, service users, families, social workers, healthcare professionals and the management team.
You will need to be confident managing competing priorities, solving problems and making sure that our care packages are appropriately staffed and supported.
- Coordinate and manage care and support packages across our services.
- Create and manage staff rotas, ensuring appropriate staffing levels and continuity of care.
- Build strong working relationships with service users, families, staff and external professionals.
- Ensure care plans, risk assessments and support documentation are up to date.
- Liaise with Social Services, healthcare professionals and other external agencies where required.
- Participate in an on-call or out-of-hours rota where required.
We are looking for someone who is organised, reliable, confident and genuinely passionate about care.
You will ideally have:
- Previous experience within a care coordination, senior care, scheduling or management position.
- Experience working within Mental Health or Supported Living settings
- Good knowledge of safeguarding and risk management.
- Excellent organisational and time-management skills.
- The ability to manage a busy workload and prioritise effectively.
- Strong written and verbal communication skills.
- Confidence communicating with staff, families and external professionals.
- Good IT skills and confidence using care management and HR systems.
- The ability to remain calm and professional when dealing with challenging situations.
- A proactive approach and the ability to work independently.
- A full UK driving licence and access to a vehicle is essential
This role is Monday to Friday 9-5 and will involve some community work as and when required.
