An exciting opportunity has arisen for a motivated and compassionate Registered Mental Health Nurse to join the Hull Community Mental Health Team (CMHT). As a Band 5 Community Mental Health Nurse, you will play a key role in delivering high quality, recovery focused care to adults experiencing complex mental health needs across community settings.
Working as part of a supportive multidisciplinary team, you will undertake comprehensive mental health assessments, develop and deliver person-centred care plans, coordinate care under the Care Programme Approach (CPA), and provide evidence-based interventions that promote recovery, independence and social inclusion. You will work closely with service users, carers, primary care colleagues and partner agencies to ensure holistic and effective care is provided.
This is an excellent opportunity for a nurse who is passionate about community mental health and wishes to develop their clinical skills within a service committed to delivering compassionate, person-centred care. You will receive regular supervision, access to ongoing professional development opportunities and support from an experienced multidisciplinary team

Manage a defined caseload of adults with complex mental health needs, providing assessment, treatment, care coordination and ongoing review.
Undertake comprehensive mental health assessments, formulate care plans and evaluate outcomes in partnership with service users and carers.
Deliver evidence-based, recovery-focused interventions that promote independence, wellbeing and social inclusion.
Assess, manage and respond appropriately to clinical risk, safeguarding concerns and crisis situations to ensure the safety of service users, carers and others
